Release, Waiver, and Consent (please read)
Gymnastics and active play involve physical activity and a risk of injury, including serious injury. In exchange for my child being allowed to participate in a birthday party at Gymtegrity Gymnastics & Cheer, 4720 Lima St., Denver, CO 80239, I agree, on my own behalf and on behalf of the child named above, that:
1. I am the parent or legal guardian of the child named above and have authority to sign for them.
2. My child is in good health and able to participate safely in gymnastics and active play.
3. I voluntarily accept all risks of my child's participation, including the risk of injury, and I release, waive, and agree not to sue Gymtegrity, its owners, employees, and coaches for any injury, loss, or damage arising from my child's participation, including injury caused by the ordinary negligence of Gymtegrity or its staff, to the fullest extent permitted by Colorado law (C.R.S. 13-22-107).
4. If my child is injured, I authorize Gymtegrity staff to obtain emergency medical care for my child, and I am responsible for the cost of that care.
5. My child will follow the coaches' instructions and gym rules, and Gymtegrity may remove any child from the activity for safety reasons.
6. Gymtegrity may use photos or video taken at the party in which my child appears for its own marketing. (Email frontdesk@gymtegrity.com to opt out.)
By checking the box below and submitting this form, I am signing this agreement electronically. It applies to this party only and remains in effect for the full party, including free play.
